Infections of The Spine: Symptoms, Risks, Diagnosis

Anyone from young children, adults, and the elderly can get a spinal infection. Individuals can have an infection:

  • In vertebral bone tissue
  • An intervertebral disc
  • The spinal canal – space where the spinal cord runs through
  • The spinal cord’s protective lining

These types of infections can happen anywhere along the spine, from the atlas at the base of the neck to the coccyx all the way to the lowest part of the back. Individuals should expect unpredictability if dealing with a spinal infection. During testing, lab results could be misleading or inaccurate. What can happen is white blood cell counts are normal, X-rays might not show any abnormalities, and sensitive diagnostic tests like a CT or MRI scan might not show positivity of infection for a week or more. Spinal Infection Types

They are classified according to the type of tissue they infect. The most common include:

Vertebral Osteomyelitis

This is a common infection type. Bacteria most often cause the cause. It can develop after trauma to the spine, post-surgery, or bacterial infections located in other body parts that travel via the blood to the vertebra. Symptoms include:

  • Persistent, chronic back pain that can become severe worsens at night and becomes aggravated from moving.
  • The pain radiates/spreads into the arms and legs
  • Tingling, numbness, and burning sensations
  • Inflammation
  • Fever
  • Weight loss
  • Vomiting
  • Post-surgery wound drainage, redness, and swelling near the surgical site

Treatment includes:

  • Intravenous antibiotics
  • Antibiotics
  • Over-the-counter analgesics like nonsteroidal anti-inflammatory meds for pain relief.
  • Prescription pain medications
  • Back brace
  • Surgery is recommended if antibiotic treatment fails, nerve damage develops, a spinal deformity develops, or to remove infected bone and/or soft tissues.

Discitis

This type of infection develops between the intervertebral discs. It is also rare, but it is more common in children and adolescents, but it can still happen in adults. Discitis can be potentially deadly, despite advanced treatment. The most common causes are bacterial and viral infections.

Symptoms
Individuals with discitis can present with minimal symptoms when the infection initializes, but it does worsen and can cause:

  • Abdominal pain and discomfort
  • Posture changes
  • Mobility issues
  • Difficulty performing everyday tasks
  • Fever
  • Severe back pain that worsens at night or by moving

Treatment options include:

  • Antibiotics
  • Anti-inflammatory medications
  • Supportive devices
  • Rest
  • Steroids to alleviate inflammation in chronic and severe cases
  • Severe cases could require surgery to restructure areas of the affected spine to improve function and mobility

Epidural Abscess

This is an infection that can develop in the spaces between the bones of the spine, the skull, or soft tissues surrounding the brain and spinal cord. This is a medical emergency that needs to be addressed immediately. The infection is often caused by a bacterial or fungal infection in or around the affected area. This is commonly a Staphylococcus aureus bacteria. In some cases, an abscess can develop from an infection in another part of the body. This could be a urinary tract infection that spreads out to the spine. Symptoms include:

  • Inflammation and swelling
  • Mid to low back pain
  • Headache
  • Fever
  • Nausea and/or vomiting
  • Neurological weakness, numbness in the arms and legs, bowel or bladder incontinence
  • Walking problems

Treatment consists of intravenous antibiotics to combat the infection that caused the abscess. In some cases, if there is difficulty moving around or are experiencing numbness, then surgery is required to drain the abscess or completely remove it.

Risk Factors

Spine infections can affect anyone. However, there are individuals with certain chronic health problems that have an increased risk. They include those with:

  • Kidney disease
  • Liver disease
  • Auto-immune diseases
  • Immune systems compromised like cancer and HIV
  • Individuals that have undergone a spinal surgical procedure are at greater risk post-surgery

Risk factors include:

  • Individuals that have had a spinal infection could be more susceptible to developing another.
  • Age – older adults
  • Malnutrition
  • Diabetes mellitus
  • Obesity
  • Smoking
  • Intravenous drug use
  • Certain areas of the back are more prone to infection. This includes the cervical/back of the neck and the lumbar/low back.

Surgical risk factors:

  • Long surgical procedure
  • Massive loss of blood
  • Multiple surgeries of the same area

Diagnosis

Early diagnosis can be the difference in treating the infection before damage to the spine develops or before the damage worsens. They are diagnosed through a variety of lab and imaging tests. These include:

  • Various lab tests
  • Blood work to examine white blood cell counts and markers for inflammation
  • X-Rays
  • CT scan
  • MRI

Outcome

These infections are rare, but they are serious and early treatment is recommended for optimal outcomes. Early diagnosis in the early stages can be successfully treated with antibiotics, rest, and spinal braces. Doctors, spine specialists, chiropractors, and physical therapists will work with the individual to provide a thorough diagnosis, personalized treatment plan, and long-term outcomes.


Body Composition


Practice Mindfulness

Practicing mindfulness can help identify triggers of negative thoughts and behavior. Mindfulness is unique to each individual. Sitting quietly and meditating for 20 minutes is not for everyone. Instead, try a five-minute guided meditation, writing, or music listening. The best time to meditate is in the morning after waking up. This helps set the day’s objectives, what’s important and what can wait in a clear fashion. Mindfulness practice has been shown to reduce stress and feelings of anxiety.

Journaling is a great way to find out about yourself. It can be done with pencil/pen and paper, on a computer, tablet, or phone. Take a few minutes to write some ideas, feelings, emotions that can help put things in perspective. Examples could be, writing something that makes you happy/proud, something that you want to improve, and a goal. There is also mindful listening that can help reduce stress by focusing attention. Instead of turning on the news or checking email first thing, listen to a favorite podcast or music. The same applies to over-phone use. During some downtime instead of scrolling through social media, etc, take a breath and listen to your mind and self.

Consuming whole-food proteins like salmon help with workout/exercise muscle protein synthesis better than a protein supplement. Omega-3’s can improve body composition because the fatty acids counteract muscle atrophy and promote recovery from muscles not being used brought on by surgery and inactivity.  
 

Cherries

Cherries contain a high source of vitamin C and polyphenols. Both contain antioxidant and anti-inflammatory properties that help reduce oxidative stress. Oxidative stress contributes to inflammation and atherosclerosis, which can contribute to cardiovascular disease. Cherries promote health, provide cardioprotective support, help to reduce blood pressure and LDL cholesterol.

Nuts are considered a nutritious superfood. They contain essential vitamins, minerals, and healthy fats. However, nuts are recommended in limited amounts because of their high-fat content. Also, nuts that are salted or flavored, lower health benefits. Individuals that eat a handful of nuts as part of a healthy diet have shown to be able to maintain and improve body composition. Healthy lifestyle diets like the DASH Diet and the Mediterranean Diet recommend moderate consumption of nuts. They help lower blood pressure, improve heart function, and reduce contributing factors to coronary heart disease and diabetes. Nuts include a variety of nutrients like vitamins:

Eating garlic has been shown to help lower cholesterol, blood pressure, delays the progression of atherosclerosis, and improves circulation. Studies have shown that garlic is highly effective in preventing yeast infections, as well as, treating ear infections. There is a compound known as allicin and helps shield the stomach from the spread of Helicobacter pylori, a bacterium related to gastrointestinal cancer.
 

Turmeric

Turmeric is another antioxidant. It helps manage inflammatory conditions like metabolic syndrome and arthritis. The polyphenol antioxidant is curcumin. It has been shown to help reduce inflammation, pain, and benefits the kidneys. Curcumin has been shown to reduce symptoms resulting from metabolic syndrome. Curcumin has been shown to:
  • Improve insulin sensitivity
  • Reduce the accumulation of fat storage that can lead to obesity

Cacao

Cacao is chocolate in the purest form. It is packed with nutrients and contains:

Cocoa is the processed form that generally contains sugars and dairy products. The beans contain antioxidants and phytochemicals like polyphenols and flavonoids that protect cells against free radicals along with oxidative stress. There is a flavanol called epicatechin and is linked to improved cognition and a better mood. Dark chocolate helps reduce metabolic syndrome risk factors, including:
  • Reduced blood pressure
  • Improved insulin sensitivity
  • Stabilized glucose levels
Heart function is improved by reducing platelet reactivity and inhibiting oxidant production.  
 

Manuka Honey

Honey has antioxidant properties and provides anti-inflammatory and anti-cancer benefits. It is made up of:

  • Enzymes

  • Proteins
  • Other compounds have anti-tumor and anti-leukemic properties
Manuka honey also has antibacterial properties and is very beneficial in healing wounds. Store-bought honey should not be used to treat cuts and wounds. Manuka honey is darker, and thicker that comes specifically from the manuka plant. It has more microbial properties than other types of honey. All honey has antioxidants, but Manuka has high levels of organic compounds that correlate with increased antibacterial activity. It is among the most powerful antioxidants that benefit athletes. Honey can easily be incorporated into a daily diet as a sweetener for tea, or as a topping for yogurt or oatmeal. However, it is not recommended for infants under one year of age. This is because babies have a higher risk of contracting botulism.

Tea

Tea contains antioxidant properties and polyphenolic compounds. Studies show the polyphenols, specifically in green tea have anti-inflammatory properties. This is a common treatment for preventing osteoarthritis. The catechins are a type of phenol that helps reduce free radicals and the presence of oxidative stress. It has also been shown to effectively improve body composition. Drinking 3-4 cups of tea a day can significantly reduce weight gain and risks associated with metabolic syndrome. However, drinking 3-4 cups a day might not be the answer for everybody, it can be something to consider.

Mediterranean Diet

This diet is based on the eating habits of those living in countries that surround the Mediterranean.   • The pain of plantar fasciitis is commonly at its worst when getting out of bed in the morning.
  • The initial steps cause a painful stretching of the tissue after hours of being in a shortened position.
  • As a person sleeps, the feet naturally go into a position of plantarflexion and shorten down in a relaxed.
  • Mild relief is common after moving about for a while.

Treatment options

Treatment options for plantar fasciitis include:

  • Reduced activity
  • Cryotherapy
  • Ultrasound
  • Taping
  • Massage
  • Chiropractic
  • Orthotics

If the patient is capable of walking without an antalgic gait, look to see how much foot flare occurs during the gait cycle.

Rule of thumb for foot flare:

  1. Both feet should be pointing in the general direction you are walking toward.
  2. Any deviation from a line directly in front involving one foot more than the other is a red flag for dysfunction in the kinetic (moving) chain and may be a contributing factor in the syndrome.
  3. Foot flare is a compensation mechanism to help balance a wobbly pelvis.
  4. If this is detected, remove shoes and socks and look at the medial arch.

  • Direct palpation of the plantar fascia may reveal painful thickened muscle, especially along the medial longitudinal arch.
  • This thickened muscle is the result of repetitive tear and repair process.
  • With the foot relaxed, grasp the toes and gently pull them up into passive dorsiflexion.
  • Gentleness is essential; as this motion stretches the irritated plantar area.
  • It often presents with fixations or subluxations of the bones of the feet.
  • The collapse of any of the three arches of the foot sets the stage for joint fixation.

Mobility & function

  • Mobility is life to a joint, so don�t overlook the benefit of adjusting the subluxations or fixations of the lower extremity.
  • Once the acute phase of plantar fasciitis is under control, you can address the factors that contributed to the condition.
  • Most of the time patients need arch structure to be supported in an optimal functioning position.
