Fractures in the cervical spine may occur with severe or repetitive trauma. Clay shoveler’s fracture is an avulsion fracture of the spinous process. It happens with abrupt flexion of the head, most commonly from a variety of accidents. It also occurs with stress caused by the pulling of the muscles around the thoracic and cervical spinous processes. The process breaks up and pulls on the avulsion segment from the original spinous procedure.
Clay shoveler’s fracture most frequently results in the lower cervical and upper thoracic spinous processes, namely C6, C7, and T1. Fractures and spinal avulsion can also be brought on by damage or injury as in direct blows to the neck’s base. This is a fracture that is stable and doesn’t produce any neurologic deficits. This part of the bone isn’t near nerve roots or the spinal cord, although patient’s are alarmed when they hear the term fracture or fracture in the neck.
Clay shovelers fracture derives its name from a common event among clay miners in Australia during the 1930s. The workers were digging deep ditches and tossing clay 10-15 feet above their heads with long handled shovels. Instead of having the clay come off the shovel, it would stick. The sticking clay produces a contraction of the trapezius and rhomboid muscles in reaction from the weighted shovel. The muscles react forcefully and immediately, contracting to stabilize the spine and shoulders. The Australian clay shoveler’s will hear a pop and feel a sharp contrast between the shoulder blades. They would feel sharp pain, making them unable to continue working because every time the trapezius or rhomboid muscles could contract. The mechanism of injury is believed to be caused by powerful and abrupt muscle contraction transmitting pressure via the ligaments. The enormous force is concentrated round the spinous processes and creates an avulsion fracture over the cervical and upper thoracic spine’s spinous processes.
Plain Film X-rays and Examination
Regarding lateral (side view) x-rays, a triangular radiolucent fracture line can be seen via the base of the spinous procedure. It is more likely to maintain the distal or trunk tip of the lower cervical and upper thoracic spinous procedure. Rough margins or serrated edges are commonly seen with the acute fractures, which differentiates it from this secondary expansion center of the process.The distal part of the fractured spinous process is frequently displaced down (caudally or inferior). This is due to the pull on the segment of bone.
Frontal x-rays (anterior to posterior) may demonstrate the look of two spinous processes at a single vertebrae, which can be called the “double spinous process sign.” Especially when the cervicothoracic junction is visualized on the view this signal is helpful for determining a clay shoveler’s fracture. An MRI (magnetic resonance imaging) or CT (computed tomography) is not typically required. A bone density scan can evaluate and measure lumbar and cervical spine t-scores; which gauges relative risk of spinal compression fractures if bone density is in question.
Symptoms of Clay Shoveler’s Fracture
Clay shoveler’s fracture can occur with almost any repetitive and forceful activity utilizing the trapezius and rhomboid muscles. It might occur with automobile accident injuries for traumatic blows to both sides and top of the spinous procedure. Generally, pain is associated immediately after the injury and can be described as a burning or “knife-like” traumatic pain. Other symptoms include muscular stiffness and pain which increases with repeated action, similar to muscle strain at the top back joints or muscle strains. The broken spine is very tender, as are the muscles.
Treatment of Cervical Spine Avulsion Fractures
Most cases resolve in a few weeks. Pain or aggravation to the area could be associated with the tendon and muscle junctions that insert on the spinous process or avulsion segment. Some patients do not require treatment apart from remainder or NSAIDS (non steroidal anti medication). Others may benefit from pain medicine or muscle relaxers.
Medications could be applied at the neck and upper back into the muscles to ease soreness and possible rhomboid muscle strains. Bones and ribs shouldn’t be influenced or produce back pain. Some individuals require physical therapy or massage therapy, including chiropractic care, to help decrease muscle pain and stiffness. Ice, heat ultrasound, mild stretching, and range of motion exercises can help relieve neck and upper back pain. Some individuals respond to course IV cold laser treatments (low level laser treatment) to help reduce pain and inflammation in the muscles and tendons. Others might benefit from muscle treatments like Active Release Technique or Graston Technique to break up scar tissue or adhesions associated with years of repetitive activities. Patients may expect some discomfort for several weeks during the healing process.
Symptoms may worsen at the the front of the body with arms and the head, such as driving or working in the computer. With time and some therapy, symptoms will decline with time. Exercise and strenuous activity might need to be avoided for 1-2 months post injury. For patient’s having a history of stress fractures or avulsion fractures, a bone density scan could be indicated. Some kinds of fractures need immediate attention and could be unstable. Fractures must be assessed to make sure they are stable and tracked by an orthopedic surgeon.

SELECTING THE RIGHT CHIROPRACTIC COLLEGE

CHIROPRACTIC SCHOOLS
Students must attend chiropractic college in order to enter the profession. Upon completion of the program, students will have earned a doctorate in chiropractic medicine. The Council on Chiropractic Education, or CCE, is the nationally recognized accrediting agency by the United States Secretary of Education which regulates the quality of the curriculum offered at chiropractic colleges. Currently there are 15 CCE accredited chiropractic institutions in the United States. These include, as listed on the CCE website:
Students attend chiropractic college for four years. During this time, students are taught the scientific and academic skills and knowledge required to become experts in the field of chiropractic medicine. The final year is spent in practice, performing the functions of a chiropractic doctor under the supervision of an experienced professional. The curriculum includes intensive study of neuromusculoskeletal conditions, nutritional and holistic health, specialized and focused curriculum in areas of acupuncture and oriental medicine, applied nutrition, and various other disciplines. Students will complete extensive course hours in diagnosis, biochemistry, anatomy, chiropractic technique, and philosophy and ancillary therapeutic procedures.
TAKING THE NATIONAL BOARD EXAM
The National Board Exam for chiropractors is administered by the NBCE. The test is given twice each year. The exam consists of three parts. Part one is 110 multiple choice questions relating to general anatomy, spinal anatomy, physiology, chemistry, pathology, microbiology, and public health. The second part also consists of 110 multiple choice questions, but in the areas of general diagnosis, neuromusculoskeletal diagnosis, diagnostic imaging, and principles of chiropractic, chiropractic practice, and associated clinical sciences. Part three of the test consists of another 110 multiple choice questions and 10 case vignettes covering the areas of diagnosis or clinical impression, clinical laboratory and special studies examination, chiropractic techniques, case management, physical examination, case history, and roentgenologic examination. Each part of the test is timed. Additional specialized testing is offered for applicants who choose to pursue an area of specialization.
LICENSING FOR CHIROPRACTORS
After successful completion of an accredited chiropractic program, graduates will need to obtain a license to practice in their resident state or the state in which they intend to practice. State licensure regulations may vary from state to state. It is important to research your state�s regulations prior to completion of the doctor of chiropractic program to ensure all conditions are met. CONTINUING EDUCATION FOR CHIROPRACTORS
The chiropractic field is experiencing an increase in advancements in technology and knowledge through research and academic exploration. Changing regulations are also an area in which chiropractic doctors will need to remain current. Each state maintains their own continuing education requirements upon which licensing will be contingent. Twenty-four credit hours of continuing education every two years is a common requirement. All programs must be board approved and conducted by approved colleges or chiropractic associations or organizations. Check with your state licensing board to determine if the program has been approved prior to enrollment.
PRACTICING AS A CHIROPRACTOR
After obtaining a doctorate and passing the licensing examination, a new chiropractor has many options ahead of them. Most chiropractors will end up working solo or in a group practice, with about one in three being self-employed. A small group will work in hospitals or physicians� offices. The median pay for Chiropractors in 2016 was $67,520, with the lowest 10 percent earning less than $32,380, and the highest earning more than $141,030. Chiropractors can further increase their salary by building up a strong client base and developing their own practice. Many times, chiropractors will work in the evening or on weekends to accommodate their patients.
DAY TO DAY PRACTICE
Chiropractors will spend a lot of time on their feet as they examine and treat patients. Some of the most important qualities that a chiropractor can have include decision-making, detail-oriented, dexterity, empathy, and interpersonal skills. If the chiropractor is operating his or her own practice, the ability to manage a staff of employees like secretaries and nurses is vital to the success of the practice. An understanding of the current healthcare system is also important, as that will determine what kind of payments a chiropractor may be able to receive, unless they work in a cash-only system. What Is A Nerve?
Nerves are your body�s messengers. They transport signals to and from your brain�messages like �Move this toe� or �Ouch, that cactus needle really is sharp.� You have a central nervous system, which is made up of your brain and spinal cord. You also have a peripheral nervous system, which is the system of nerves that branches off the brain and spinal cord.
What Do Nerves Look Like?

What Can Pinch a Nerve?
As a nerve exits the spinal canal, it can be pinched by a herniated disc or a bone spur. Bone spurs, also known as osteophytes, are bony bumps that can develop on a spinal joint over time. They can push into the spinal nerve, as you can see in this illustration (red = pain generator, of course).
What Does a Pinched Nerve Feel Like?
A pinched nerve mostly feels like pain. If you have a pinched nerve in your low back, it can cause pain to travel (or radiate, in doctor-speak) down your leg. You may also know that as sciatica. A pinched nerve in the neck can create pain that shoots down your arm. Other symptoms of pinched nerves include muscle spasms, burning, tingling, and a hot/cold sensation.
Now the Good Stuff: Pinched Nerve Treatments
Pinched nerve treatments fall into two categories: what you can do at home (self-care) and what your doctor may prescribe for you.
Pinched Nerve Self-care #1: Heat and Ice
Heat and ice can work wonders on a pinched nerve. Switch between 20 minutes of heat and 20 minutes of ice�and remember that you shouldn�t put the heat and ice packs directly on your skin.
