Lumbar Traction

Lumbar traction therapy for lower back pain and sciatica could be a treatment option to help restore mobility and flexibility and safely support an individual’s return to an optimal level of activity. It is often combined with targeted therapeutic exercise. (Yu-Hsuan Cheng, et al., 2020) The technique stretches the space between the vertebrae in the lower spine, relieving lower back pain.

  • Lumbar or low back traction helps to separate the spaces between the vertebrae.
  • Separating the bones restores circulation and helps relieve the pressure on pinched nerves like the sciatic nerve, decreasing pain and improving mobility.

Research

Researchers say lumbar traction with exercise did not improve individual outcomes compared to physical therapy exercises on their own (Anne Thackeray et al., 2016). The study examined 120 participants with back pain and nerve root impingement who were randomly selected to undergo lumbar traction with exercises or simple exercises for pain. Extension-based exercises focused on bending the spine backward. This movement is considered effective for individuals with back pain and pinched nerves. The results indicated that adding lumbar traction to physical therapy exercises did not offer significant benefits over extension-based exercise alone for back pain. (Anne Thackeray et al., 2016)

A 2022 study found that lumbar traction is helpful for individuals with lower back pain. The study investigated two different lumbar traction techniques and found that variable-force lumbar traction and high-force lumbar traction helped to relieve lower back pain. High-force lumbar traction was also found to reduce functional disability. (Zahra Masood et al., 2022) Another study found lumbar traction improves the range of motion in the straight leg raise test. The study examined different forces of traction on herniated discs. All the levels improved the individuals’ range of motion, but the one-half body-weight traction setting was associated with the most significant pain relief. (Anita Kumari et al., 2021)

Treatment

For individuals with only low back pain, exercise, and postural correction may be all that is needed to provide relief. Research confirms physical therapy exercises can help decrease pain and improve mobility (Anita Slomski 2020). Another study revealed the importance of centralizing sciatic symptoms during repetitive movements. Centralization is moving the pain back to the spine, which is a positive sign that the nerves and discs are healing and occurs during therapeutic exercise. (Hanne B. Albert et al., 2012) A chiropractor and physical therapy team can educate patients on preventing back pain episodes. Chiropractors and physical therapists are body movement experts who can show which exercises are best for your condition. Starting an exercise program that centralizes symptoms can help individuals return to their normal lifestyle quickly and safely. Regenerative Cells

Regenerative cells are stem cells that are specialized to potentially develop into many different types of cells. They are unlike any other cell because:

  • Being unspecialized they have no specific function in the body.
  • They can become specialized cells like – brain, muscle, and blood cells.
  • They can divide and renew continually for a long period.
  • Blood stem cells are currently the only type that is regularly used in treatment.
  • For leukemia or lymphoma, only adult cells are used in a procedure known as a bone marrow transplant. (Cleveland Clinic. 2023)
  • For regenerative cell research, the cells can come from different sources, including adult donors, genetically altered human cells, or embryos.

Bone Marrow Transplants

  • Bone marrow cells produce all of the body’s blood cells, including red and white blood, and platelets.
  • Hematopoietic stem cells are those found in bone marrow that is the parent for the different types of cells.
  • Hematopoietic cells are transplanted in individuals with cancer to replenish bone marrow.
  • The procedure is often used during high-dose chemotherapy that destroys the existing cells in the bone marrow.
  • Donated stem cells are injected into a vein and settle in the bone marrow where they begin to produce new healthy blood cells. (Cleveland Clinic. 2023)

Peripheral Blood Transplants

  • For some time the only source to extract hematopoietic cells was from bone marrow.
  • Researchers found that many of these cells were freely circulating in the blood.
  • Scientists learned how to extract the cells from the blood and transplant them directly.
  • This type of transplant is a peripheral blood stem cell transplant/PBSCT and has become the more common procedure, however, both methods are still used. (Cleveland Clinic. 2023)
  • PBSCT is less invasive and does not require the removal of marrow from the hip bone.

Embryonic stem cells were first grown in a laboratory in 1998 for reproductive research, which created controversy because they were extracted from human embryos that were destroyed or harvested for science. Today their primary use is for research into treatments and potential cures for:

Embryonic cells are pluripotent, which means they can grow into the three types of germ cell layers – ectoderm, mesoderm, and endoderm – that make up the human body and can develop into each of the more than 200 types of cells. (National Institutes of Health. 2016)

Induced Pluripotent Cells

  • Induced pluripotent stem cells/iPSCs are somatic cells that have been genetically reprogrammed to behave like embryonic cells.
  • iPSCs are usually skin or blood cells that undergo genetic programming. (Pavel Simara, Jason A Motl, Dan S Kaufman. 2013)
  • iPSCs were first developed in 2006.
  • A major advantage of iPSCs over somatic and embryonic cells is that iPSCs can be generated to match the patient.
  • This means that a laboratory can tailor-make cells from an individual’s own cells or tissues.

Non-Binary

Non-binary is a term used that describes individuals who do not identify exclusively as male or female. The term addresses various gender identities and expressions that are outside of the traditional gender binary system, which categorizes individuals as either male or female.

Definition

  • Non-binary individuals are those whose gender identity and/or expression fall outside of the traditional binary categories of a man or woman. (Human Rights Campaign. (n.d.))
  • Some non-binary individuals identify as a blend of male and female; others identify as a gender different from male or female; some do not identify with any gender.
  • The term “non-binary” can also be “enby”/phonetic pronunciation of the letters NB for non-binary, although not every non-binary individual uses this term.

Genderqueer

  • An individual who does not follow conventional gender norms.

Agender

  • An individual who does not identify with any gender.

Genderfluid

  • An individual whose gender identity is not fixed and can change over time.

Demigender

  • An individual who feels a partial connection to a particular gender.

  • An individual who identifies as both male and female or a combination.

Pangender

  • An individual who identifies as many genders.

Androgynous

  • An individual whose gender expression is a mix of masculine and feminine traits or…
  • Who identifies as having a gender that is neither male nor female.

Gender Nonconforming

  • An individual who does not conform to societal expectations or norms of gender expression or identity.

Transgender/Trans

  • An individual whose gender identity differs from the gender assigned at birth.

Non-Binary Pronouns

A pronoun is a word used to replace a noun.

  • In gender context, pronouns refer to an individual without using their name, like “he” – masculine or “she” – feminine.
  • Non-binary individuals may use pronouns that do not fit the pronoun associated with the gender assigned at birth.
  • Instead, they will use pronouns that more accurately reflect their gender identity.
  • They/them” are gender-neutral pronouns that refer to someone without assuming their gender identity.
  • Some non-binary individuals use “they/them” pronouns, but not all.
  • Some may use “he/him” or “she/her” or a combination.
  • Others may refrain from using pronouns and instead ask you to use their name.

Transgender individuals and non-binary individuals are two distinct groups that are related.

  • There are some transgender/trans individuals who are non-binary, however, most transgender individuals identify as either male or female. (National Center for Transgender Equality. 2023)
  • To understand the difference, it can help to know the meanings of transgender, cisgender, and nonbinary: (GLAAD. 2023)

Transgender

  • An individual who identifies with a gender different from the one assigned at birth.
  • For example, someone assigned male at birth/AMAB, but identifies as a female is a transgender woman.

Cisgender

  • An individual whose gender identity follows the one they were assigned at birth.
  • For example, someone assigned female at birth/AFAB and identifies as a woman.

Non-binary

  • An individual who identifies with a gender outside the traditional binary of male and female.
  • This can include individuals who identify as genderqueer, agender, or genderfluid and others.

Using Pronouns

Using non-binary pronouns is a way to show respect and validation for an individual’s gender identity. Here are some recommendations on how to use pronouns: (National Center for Transgender Equality. 2023)

Ask for the individual’s pronouns

  • It’s recommended to avoid assuming an individual’s pronouns based on appearance or stereotype.
  • If unsure of someone’s pronouns, ask respectfully.
  • “What pronouns do you use?”
  • “Can you share your pronouns with me?”

Practice using the pronouns

  • Once you know an individual’s pronouns, practice using them.
  • This can be accomplished by using their pronouns when referring to them in conversation, emails, written forms, and/or other types of communication.
  • If you make a mistake, apologize and make the correction.

Gender-neutral language

  • If unsure of an individual’s pronouns, or if someone uses gender-neutral pronouns like they/them, use gender-neutral language instead of gendered language.
  • For example, instead of saying he or she, you can say they or their name.

The lymphatic system is a network throughout the body. The system drains lymph fluid from the blood vessels into the tissues and empties it back into the bloodstream through the lymph nodes. The main functions of the system include the following:
