Treatments

Back Clinic Treatments. There are various treatments for all types of injuries and conditions here at Injury Medical & Chiropractic Clinic. The main goal is to correct any misalignments in the spine through manual manipulation and placing misaligned vertebrae back in their proper place. Patients will be given a series of treatments, which are based on the diagnosis. This can include spinal manipulation, as well as other supportive treatments. And as chiropractic treatment has developed, so have its methods and techniques.

Why do chiropractors use one method/technique over another?

A common method of spinal adjustment is the toggle drop method. With this method, a chiropractor crosses their hands and pressed down firmly on an area of the spine. They will then adjust the area with a quick and precise thrust. This method has been used for years and is often used to help increase a patient’s mobility.

Another popular method takes place on a special drop table. The table has different sections, which can be moved up or down based on the body’s position. Patients lie face down on their back or side while the chiropractor applies quick thrusts throughout the spinal area as the table section drops. Many prefer this table adjustment, as this method is lighter and does not include twisting motions used in other methods.

Chiropractors also use specialized tools to assist in their adjustments, i.e., the activator. A chiropractor uses this spring-loaded tool to perform the adjustment/s instead of their hands. Many consider the activator method to be the most gentle of all.

Whichever adjustment method a chiropractor uses, they all offer great benefits to the spine and overall health and wellness. If there is a certain method that is preferred, talk to a chiropractor about it. If they do not perform a certain technique, they may recommend a colleague that does.

After all of these years, I am happy to announce that the Texas Supreme Court has finally made a decision regarding the Texas Board of Chiropractic Examiners et al v. Texas Medical Association case on January 29th, 2021. With great honor and gratitude, I’d like to continue to extend sincere thanks to everyone who worked hard on this case and whose tremendous efforts resulted in the decision. Thanks to the Supreme Court’s decision, chiropractors in Texas can now carry on their jobs accordingly. Below, I have provided a letter from Board President, Mark R. Bronson, D.C., F.I.A.N.M. on behalf of the Texas Board of Chiropractic Examiners stating the Texas Supreme Court’s decision in the Texas Board of Chiropractic Examiners et al v. Texas Medical Association case on January 29th, 2021. – Dr. Alex Jimenez D.C., C.C.S.T.

On behalf of the Texas Board of Chiropractic Examiners, I extend our sincere thanks and appreciation to everyone whose efforts resulted in the Texas Supreme Court’s decision in Texas Board of Chiropractic Examiners et al v. Texas Medical Association on January 29, 2021. Special thanks are due to all the attorneys at the Office of the Attorney General who worked on this case over these years.

 

The decision properly affirmed the validity of the Board’s scope of practice rule, which the court clearly said does not exceed our statutory scope of chiropractic practice. The court unequivocally held that the Board�s rules do not violate Occupations Code Chapter 201 or run counter to the chapter’s objectives set by the Texas Legislature, and in fact, carefully observe the statutory boundary between the medical and chiropractic professions. This decision, which recognizes the common sense and long-standing inclusion of associated nerves in chiropractic diagnosis and treatment, preserves and strengthens the essence of chiropractic.

 

Thanks to the court’s decision, our licensees can now fulfill their duties as vital portal-of-entry healthcare providers in Texas without fear. The court’s decision reaffirms the principles of economic freedom that have made Texas the best state in the nation to be a chiropractor.

Extinguish Chronic Inflammation With Chiropractic

Chronic inflammation is quite common and can cause a variety of painful back conditions. Fortunately, chiropractic treatment can extinguish the inflammation at its core/root cause and bring long-lasting relief. Inflammation can be a significant factor in causing body and back pain. The proper and lasting treatment goes beyond anti-inflammatory medications. The objective is to extinguish the inflammation completely.  
 

Inflammation

The number of diseases associated with chronic inflammation is increasing exponentially. An estimated 60% of Americans deal with at least one chronic condition caused completely or in part by inflammation. Inflammation is the body’s immune response to something that is not right in the body. Typically, there is:
  • Swelling
  • Redness
  • Warmth/heat signaling an inflammatory response from injury or infection
Chronic inflammation can present through a series of symptoms, including:
  • General fatigue
  • Pain throughout the body
  • Off feeling/not focused
Inflammatory conditions can include:
  • Skin changes like a rash
  • Joint swelling
  • Lymph node swelling
  • Endocrine, heart, lung, and neurological complications
 
Inflammation can generate inflammatory back pain as well when this happens. This is because the immune system is trying to protect the body from whatever is trying to infiltrate. The cells and chemicals fight the attack and try to heal whatever is going on. Sometimes, there is nothing going on but the immune system becomes hyper-reactive and attacks itself. This is the case of autoimmune disease, or what becomes chronic inflammation. Chronic inflammation lasts longer than 3 months and can lead to diseases like:
  • Diabetes
  • Heart disease
  • Autoimmune inflammation can manifest in rheumatological disorders like rheumatoid arthritis

Inflammation and Inflammatory Conditions

Autoimmune inflammation that presents as ankylosing spondylitis and rheumatoid arthritis is in a different category. This is when the body begins attacking itself. This can be triggered by a viral infection, which stimulates the immune system. This can cause:
  • Joint destruction
  • Ligament pain
  • Soft tissue swelling
  • Some conditions are hereditary

Inflammatory Spinal Conditions

Inflammation can travel into the body�s spinal and neurological systems. This results in painful conditions that can affect an individual’s quality of life.

Ankylosing Spondylitis

Ankylosing spondylitis usually starts in the lower back and can spread up. It is a form of arthritis that causes the vertebrae to fuse together. It can also cause inflammation in the urological and ophthalmological systems. It is believed to have a somewhat genetic connection. There is a marker called HLA B27 that is usually positive in patients and is more common in young males.

Rheumatoid Arthritis

Rheumatoid arthritis/RA causes inflammation in synovial joints. These produce a fluid that helps lubricate and nourish the joints. RA is found most commonly in the hands, wrists, and knees, but can also present in the spine�s facet joints that connect vertebrae. There is some genetic association but is common with smoking and obesity. It is diagnosed with lab work, like inflammation markers, rheumatoid factor, and physical examination. RA pain typically presents in the cervical spine or neck region.

Transverse Myelitis and Multiple Sclerosis

These conditions are closely tied and caused by inflammation in the central nervous system/CNS. The immune system attacks the nerve cells and removes the fatty substance that insulates the nerves and helps them transmit impulses to and from the central nervous system. This causes:
  • Pain
  • Weakness
  • Numbness
  • Bladder/bowel issues
Transverse myelitis affects the spinal cord whereas multiple sclerosis can affect the brain and the spinal cord. Transverse myelitis is typically acute, whereas multiple sclerosis is long-term and can have an increasing/decreasing course of progressive symptoms. And transverse myelitis can be a symptom of multiple sclerosis. Certain lifestyle choices/habits can cause or worsen inflammation. Obesity, smoking, and an unhealthy diet can have a significant impact on chronic inflammation.  

Spinal Structures Affected

Inflammation can affect every area of the spine. From the lower back to inflammation of the vertebrae themselves. Injuries to the spine, that include the:
  • Bones
  • Discs
  • Ligaments
  • Joints
This can cause swelling and a build-up of fluid that can be found on an MRI. This is the preferred method of detecting inflammation. When the term itis is used this usually indicates a type of inflammation. For example, neuritis means inflammation of the nerve/s. This can be seen with nerve compression where the nerve is swollen on an MRI.

Extinguish Spinal Inflammation

Causes of inflammation can be traced back to lifestyle choices, but can also be alleviated through healthy lifestyle adjustments that turn into habits.

Nutritional Health Coaching

Recommended is to avoid or decrease processed foods, trans fats, and sugar. Consider supplementing vitamin D, magnesium, and omega 3s.

Quit Smoking

Quitting smoking will improve circulation and extinguish vascular inflammation.

Regular Physical Activity/Exercise

Aerobic exercises improve cardiac function and circulation as well as exercises that support the spine ergonomically. Core and pelvic stabilization are necessary for lower back pain.

Medication

For acute inflammation caused by injuries steroids and NSAIDs can help with severe pain and inflammation.  
 

Chiropractic Extinguisher

When the spine and the body’s joints are in proper alignment, and the nerves are functioning at optimal levels the body’s biomechanics return to normal. This stops neuropeptides from producing and helps extinguish inflammation.

Surgery

With the spine, surgery is not recommended as a first-line treatment unless it is an emergency or there is potential for permanent neurological damage. If medication, chiropractic, physical therapy, supplements, lifestyle changes, and/or complementary treatments like acupuncture do not help and quality of life and function are significantly impacted, then elective surgery can be considered.

Whiplash is one of the most devastating cervical/neck spinal injuries. Radiculopathy

Radiculopathy is the result of a pinched nerve. This sends pain signals throughout the entire length of the nerve. With the neck’s nerve bundles, this means the pain travels as far as the face and hands. Tracing the extent of the radiculopathy informs the chiropractor as to which specific vertebrae have been affected. For example, if the nerve bundle in C7, which is the lowest neck vertebrae gets pinched, it can present in the arms. Another example, if chronic and persistent headaches or cheek pain are presenting, then C3-C4 will be the focus. The key is to look past the site of the pain and find its point of origin. Tracing referred pain quickly will allow the chiropractor to get a handle on the symptoms, even after the accident.

Skeletal muscle is one of three major types of muscles in the body. The others are smooth and cardiac muscle. These muscles are attached to the bone by tendons. Skeletal muscles consist of nerves and blood vessels along with connective tissue to operate properly. These muscles consist of cells that come together and form bundles of skeletal muscle fibers. Strength training stimulates the muscle fibers and combined with proper nutrition causes the muscles to grow. Muscles contract and shorten pulling on the bone and joints. This is what causes body movement. The nervous system sends signals to the nerves in the muscle and trigger contractions. Besides movement, skeletal muscle helps maintain posture, generate body heat, and stabilizes the bones and joints.

Steps to Control Stress

 

Take frequent physical and mental breaks

Throughout a day, there can be several and various stressful situations. Not letting the stress build-up is a skill that needs to be practiced. Every 50 minutes of work, take a 10-minute break to reduce any stress. This will increase productivity. In the middle or last break, practice deep breathing.

  • Inhale slowly until the air reaches the bottom of the stomach.
  • Exhale slowly like a balloon slowly releasing the air.
  • To achieve the full potential of deep breathing, every exhalation should last twice as long as each inhalation.
 

Caffeine reduction

Caffeine can be medicinal in small doses. It increases bile flow. This stimulates the gall bladder and reduces cholesterol. However, too much can interfere with the body’s systems that are responsible for stress response. Try to limit to one to two caffeinated drinks a week.

 

Regular physical activity

Physical activity is what helps remove waste products, specifically lactic and uric acids, from the muscles and nervous system. An excess of these acids makes the body edgy. Moderate activity prevents and reduces anxiety. Try to get in at least 10 to 15 minutes of activity in the morning and afternoon. A quick walk combined with deep breathing is perfect.

 

Plenty of sleep

Hard work and playing hard help the body to sleep to the fullest. An irregular sleep pattern or poor-quality sleep confuses the body. The body reacts abnormally. Examples like confusion, tension, an adrenaline rush kicks in when it is not needed. Chronic poor sleep makes the body more vulnerable to disease and chronic conditions.

Natural Medicine Supplements

Natural medicine supplements have been shown to reduce mild to moderate stress and anxiety.

  • Before trying any vitamins, minerals, or herbal therapies check with a doctor. This is to ensure safety with any medical conditions like pregnancy, that could prevent an individual from taking certain supplements.
  • Also, make sure if taking any medications that could cause natural medicine supplements ineffective or create a dangerous situation.
  • The reason for this is if an individual takes several supplements and develops side effects, it becomes a challenge to figure out which is responsible.
  • Quality herbs and supplements can be purchased from a licensed naturopath physician, nutritionist, health coach, or from a health store.

Herbal supplements listed below can be taken for three months and then a two-week break should be implemented. This minimizes the potential for negative effects.

 

Vitamin B

The B vitamins, especially B6 and B12 help regulate the body’s response to stress and keeps the nervous system healthy.

  • Vitamin B complex formula that has at least 25 mg of B6 and also 1 mg of B12 can be taken twice a day.
  • These vitamins are optimized when used in combination with other nutrients like a multivitamin-mineral supplement also twice a day.
  • B12 in the form of hydroxocobalamin or methylcobalamin is better absorbed than the more common cyanocobalamin.

Kava

This is herbal supplement helps with anxiety and insomnia. It can help relieve acute and chronic anxiety without causing drowsiness or brain fog. Kava is effective for settling anxiety with no dependence potential. Kava should be avoided if there is regular alcohol consumption or if taking Valium, Xanax, sleeping pills, like Seconal or Halcion, or anti-depressants, like Prozac or Zoloft.

 

Valerian

This is a natural sedative. It is used for relieving anxiety because it helps with sleep. The ingredients are similar to Valium. It is not addictive and does not cause grogginess the next day like sleeping pills. Recommended intake is one 150mg capsule twice a day. However, low doses can cause drowsiness. Therefore it is best used for sleep, one 300mg capsule one hour before sleeping is recommended. Do not use if taking sedatives, like phenobarbital or benzodiazepines.

Achieving healthy body composition is more than just building muscle and losing fat. Proper diet and getting enough sleep are necessary to make meaningful progress. With chronic stress or poor sleep, this cannot be achieved. Depressive symptoms can be associated with obesity, and anxiety and depression can trigger emotional eating and weight gain. Proper stress management means making sure there are positive strategies, techniques that can be turned to in time of need. Without a plan, stress will cause health problems and will be a significant barrier when trying to achieve fitness and health goals.

Testing For Piriformis Syndrome or Sciatica through Chiropractic

Identification of piriformis syndrome or sciatica requires proper testing and examination. The piriformis muscle begins at the sacrum near the sacroiliac joint and is attached to the femur/thigh bone at the outer area of the hip. The sciatic nerve passes typically under or through the muscle before going down the back of the thigh.

The piriformis helps turn the hip outward and bring the thigh outward to one side while the hip is bent. This could be when raising the knee and bringing the leg out. An example is stepping out of a car. The muscle also helps to stabilize when walking, running, and standing. Individuals with the sciatic nerve passing through the piriformis have an increased chance of developing piriformis syndrome. It can also be called piriformis sciatica since it is not true sciatica.  

 

Symptoms

Piriformis syndrome does not always present the same way. Common symptoms include pain, tingling, and numbness in the buttocks that becomes worse when sitting. Other symptoms can include:

  • Pain when sitting, standing, or walking.
  • Pain when getting up from a seated/squatting position
  • Pain in the sacroiliac joint
  • Pain and/or pins and needles, burning, tingling, or itching sensation
  • Movement helps to reduce pain symptoms
  • Numbness in the foot

Many of these symptoms can be mistaken for sciatica. This is why proper testing is necessary to provide the appropriate treatment; otherwise, the condition could worsen or create new injuries.

 

Causes

  • Primary piriformis syndrome happens when splitting the piriformis muscle, the sciatic nerve, and/or the sciatic nerve does not run along the normal path.
  • Secondary piriformis syndrome is more common and is caused by inflammation of the soft tissues, muscle spasms, etc. And the result is nerve compression.

  • Direct trauma to the buttock area can definitely cause inflammation, scarring, and contraction of the piriformis muscle. This could be the result of an automobile accident or a fall.
  • The most common cause is a progressive tightening of the muscle brought on from a weakened piriformis muscle.

 

Testing

Because of the close relation between piriformis syndrome and sciatica, a chiropractic medical professional will perform various tests to determine if symptoms are spinal disc-related or caused by the sciatic nerve getting pinched or impinged piriformis muscle. A chiropractor will examine the low back, hip, pelvis, sacroiliac joint, walking gait, posture, and leg length. They will test various body reflexes as well. Other tests can include:

  • Palpation/manipulation of the piriformis muscle
  • A Straight leg raise will be done to see if there is localized pain when pressure is applied to the piriformis muscle and the tendon. The chiropractor will flex the hip at a 90-degree angle and extend/straighten the knee.
  • A Freiberg test will be done to see if pain presents around the piriformis or reproduces symptoms.
  • The Pace maneuver test looks for pain and/or weakness during rotation of the hip when sitting.
  • The F.A.I.R test stands for flexion, adduction, and internal rotation. This test has the individual lie on the non-affected side while the chiropractor guides the painful leg into hip flexion, turns it in toward the body, and gently turns the lower leg outward.
  • The Beatty maneuver tests for pain while lying on the non-affected side, and the chiropractor elevates the flexed symptomatic leg.
