Nrf2 also plays a fundamental role in the management of mitochondrial function and cellular proteostasis, which suggests the possible benefits of Nrf2 to control neurodegenerative disorders. When under stress, Nrf2 activates the transcriptional upregulation of a large network of cytoprotective genes that allow adaptation and survival. The levels and activity of Nrf2 are controlled through ubiquitination and proteasomal degradation mediated by several ubiquitin ligase systems, including Keap1-Cul3/Rbx1, ?-TrCP-Cul1, and Hrd1. Keap1 is the most well understood key regulator of Nrf2. �
Keap1 functions as a primary sensor for electrophiles and oxidants, which chemically change certain cysteines in Keap1, leading to conformational changes that protect Nrf2 from Keap1-associated degradation. Subsequently, Nrf2 will accumulate and then translocate to the nucleus where it binds as a heterodimer with a small Maf transcription factor to antioxidant response elements in the promoter of its target genes to activate the expression of a large network of detoxification, antioxidant, and anti-inflammatory genes as well as other genes involved in the clearance of damaged proteins. Of particular interest is also the upregulation of genes responsible in the biosynthesis and the regeneration of glutathione (GSH), a major intracellular antioxidant. Moreover, Nrf2 also suppresses proinflammatory responses through transcriptional repression and it is involved in the regulation of mitochondrial function. The symptoms of sporadic ALS and familial ALS are similar, which suggests the involvement of common pathogenic mechanisms, including oxidative stress and neuroinflammation. �
Research studies show that oxidative stress and neuroinflammation should be the key therapeutic targets of Nrf2 signaling in ALS. Genetic research studies in ALS mouse models have shown a considerable therapeutic effect of increased Nrf2 levels in astrocytes, the main GSH producers for neurons. Furthermore, Nrf2 signaling is fundamental for controlling neuroinflammation in ALS through the management of the effects of activated microglial cells on overall neuronal survival. Consistent with the therapeutic potential of Nrf2 signaling, treatment with small molecule activators, including the extremely potent cyanoenone triterpenoids, has ultimately shown efficacy in research study mouse ALS models. �
The neuroprotective potential of Nrf2 activation has been evaluated in experiments utilizing genetic mouse HD models. HD is an autosomal dominant and highly penetrant neurodegenerative disorder, which results from the pathological expansion of a trinucleotide CAG repeats encoding polyglutamines in HTT protein. Complex pathogenic mechanisms have been shown in HD, however, excessive oxidative stress has been recognized as a fundamental driver of pathology. The harmful role of oxidative stress has been described in both HD patients and in experimental clinical trial models and it is potentially due to the neuronal sensitivity of an excess in ROS. The levels of several Nrf2-dependent antioxidant proteins, including glutathione peroxidases, catalase, and superoxide dismutase 1, are increased in human HD brains as compared with non-disease controls, suggesting a partial activation of Nrf2 defense signaling yet insufficient enough to block progressive neurodegeneration. Pharmacological activation of Nrf2 results in the broad antioxidant effects of HD in mouse brains and ameliorates the neurological phenotype. Increased expression of several key inflammatory mediators has been shown in the blood, the striatum, the cortex, and the cerebellum from postmortem patient HD tissues, however, neuroinflammation in HD patients seems to be less pronounced than in ALS or PD patients. �
Neurological Disease Treatment with Nrf2 Activation
Finally, the neurological phenotype of the most common neurological disease and neurodegenerative disorder, PD, can be challenged by Nrf2 activation. PD is characterized by the progressive loss of dopaminergic neurons in the substantia nigra and the profound reduction of dopamine in the striatum. Currently available dopaminergic treatments offer relief from several symptoms but these only address the motor manifestations. Multiple genetic and environmental factors have been suggested in the etiology of PD, however, like ALS, the majority of the clinical cases are sporadic. The discovery that the environmental neurotoxin 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) causes Parkinson’s disease in humans, led to the development of the MPTP mouse model of disease, which to date, remains one of the most highly utilized animal models of sporadic PD, including the evaluation of drug efficiency. Nrf2 activators showed neuroprotective effects in MPTP mice, which are associated with a reduction of oxidative damage and neuroinflammation. The identification of causative mutations in SNCA, the gene encoding ?-synuclein (aSyn), developed genetic mouse PD models, in which daily oral delivery of the Nrf2 activator dimethyl fumarate (DMF) protected nigral dopaminergic neurons against aSyn toxicity. �
Although oxidative stress and neuroinflammation are pathological hallmarks of AD, a therapeutic role of Nrf2 signaling has developed more slowly, perhaps due to the complexity of disease pathogenesis and readouts of efficiency. Nonetheless, there is a number of recent research studies that demonstrate the efficiency of Nrf2 activators in AD mouse models. � DMF, a U.S. FDA-approved drug (Tecfidera, Biogen-Idec) for the treatment of relapsing multiple sclerosis, activates Nrf2 through the regulation of the Keap1 sensor. DMF is of seemingly low potency and specificity, which prevents neurodegeneration. Drug-like molecules with a similar mechanism of action or with the ability of direct interference with the Keap1/Nrf2 interaction are emerging. The data demonstrate the feasibility to develop Nrf2 activators for treatment. �
In summary, although the causes and symptoms of neurological diseases are different, neurodegenerative disorders share similar molecular mechanisms, which could be regulated and managed with Nrf2 activators. Moreover, targeting Nrf2 signaling may offer a safe and effective treatment approach for a variety of these health issues. Because pharmacological Nrf2 activation targets the broad mechanisms of these health issues, all neurodegenerative conditions would be eligible for treatment. If you are experiencing any of these situations, then you might be experiencing fibromyalgia.
Fibromyalgia is a common and chronic syndrome that causes pain and mental distress in the body. It causes widespread musculoskeletal pain, and it is accompanied by fatigue, sleep memory, and mood issues to the body.�The symptoms may be similar to arthritis; however, fibromyalgia is a rheumatic condition and causes soft tissue pain or myofascial pain.
There are three symptoms that fibromyalgia causes a person to have discomfort in their daily lives. They are:
Widespread pain: This pain is associated with fibromyalgia, and it is described as a constant dull ache that can last for at least three months. For it to be considered as widespread pain, it must occur both sides of the body, as well as above and below the waist.
Fatigue:Individuals with fibromyalgia often awaken tired, even though they have been sleeping for long periods. The pain that fibromyalgia causes can disrupt the person’s sleep patterns, causing them to have sleep disorders such as restless legs syndrome and sleep apnea.
Cognitive difficulties: This symptom is commonly known as �fibro fog.� It impairs the person�s ability to focus, pay attention, and concentrate on mental tasks.
Other symptoms can include:
IBS (irritable bowel syndrome)
Stiff joints and muscles in the morning
Headaches
Problems with vision
Nausea
Pelvic and urinary problems

In the past, studies have shown that patients diagnosed with fibromyalgia had 11 out of 18 specific trigger points all around their bodies. Healthcare providers would check their patients and document how many of these points were painful to their patients by firmly, but gently, pressing their bodies to get a diagnosis.

The back of the head
The tops of the shoulders
The upper chest
The hips
The knees
The outer elbows
Nowadays, in a 2016 revised diagnostic criteria, healthcare providers can diagnose patients with fibromyalgia if the patient has pain in 4 out of 5 areas of the body that is causing them pain. The protocol when diagnosing patients is referred to as �multisite pain.�
Fibromyalgia Affecting the Endocrine System
When it comes to fibromyalgia, the symptoms are associated with the endocrine system. Research shows that fibromyalgia-like symptoms such as muscle pain and tenderness, exhaustion, reduced exercise capacity, and cold intolerance can resemble symptoms that are associated with the endocrine dysfunction like hypothyroidism and adrenal or growth hormone insufficiency.
More research has stated that fibromyalgia causes a person to develop chronic fatigue syndrome. Chronic fatigue syndrome causes the body to have a deficiency of serotonergic activity and the hypofunction of sympathetic nervous system function that could contribute to the abnormalities of the central components of the HPA axis. It can distort the body’s hormonal pattern that is being attributed to the hyperactivity of the CRH neurons. The hyperactivity caused by the CRH neurons can be driven and sustained by stress being exerted by chronic pain that has originated in the musculoskeletal system or the alternation of the central nervous system mechanism of nociception.
Researchers believe that repeated nerve stimulation causes the brains of patients with fibromyalgia to change. The change causes an abnormal increase level of certain chemicals (neurotransmitters) in the brain that signals pain. In addition, the brain�s pain receptors will develop a sort of memory of the pain that is causing problems to the patient�s body and causing them to be more sensitive since the signals are overreacting.
Treating Fibromyalgia
Even though fibromyalgia pain can be uncomfortable and consistent enough to interfere with a person�s daily routine. There are ways to relieve the pain and inflammation that fibromyalgia causes the body. Pain medication can bring down the inflammation and help a person sleep a little better. Other safe treatments that can help manage fibromyalgia pain are:
Acupuncture: Acupuncture is a Chinese medical system that uses needles to cause changes in the blood flow and the levels of neurotransmitters in the brain and spinal cord.
Therapy: A variety of different therapies can help a person reduce the effects that fibromyalgia has caused.
Yoga and tai chi: These practices combine meditation, slow movements, deep breathing, and relaxation � both help control fibromyalgia symptoms.
Reducing stress: Developing a plan to avoid or limit overexertion and emotional stress is useful when dealing with fibromyalgia. Learning to meditate and trying stress management techniques can help a person feel calm and recharged for the rest of the day.
Getting enough sleep: Since fatigue is one of the main characteristics of fibromyalgia, getting enough sleep is essential. Practicing good sleep habits and going to bed and getting up at the same time each day can lessen the effects of fatigue.
Exercising regularly: At first, exercising may increase the pain, but doing it gradually and regularly over time can decrease the symptoms. This can be walking, swimming, biking, and water aerobics can be beneficial to the body.
Pacing yourself: Keeping track of activities is beneficial for people with fibromyalgia. Moderation of daily activities on the good days can help a person overcome the symptoms when they flare-up.
Maintaining a healthy lifestyle: Eating healthy food that has anti-inflammatory properties can be useful for the body, and finding enjoyable hobbies can be beneficial as well.

Fibromyalgia is a chronic illness that causes pain and inflammation that affects the soft tissue in the body. Health Coaching is a recent position that is being utilized by doctor offices around the country. Many doctors have realized that their patients are needing more one on one guidance but they are unable to provide this due to their hectic schedule. This is where they have created and utilized health coaches.
Health coaching is extremely beneficial for patients and can help them achieve their health goals. For more information about health coaches and a general overview of the essential role they play in the healthcare field, please see last week’s article linked here.
Health coaches use many different techniques depending on the patient they are working with, but the core values of their methods remain the same. These core values can be broken down into 6 different steps, with each individual step having smaller more detailed steps of their own. These steps can be identified as:
Identifying values and vision
Determining goals
Building a plan for action
Tracking progress
Visualizing one’s best self
Creating a plan for resiliency
Step 1: Identify Values
With this step being the first, it is one of the most crucial. When a patient comes to a physician or a health coach, it is usually because they have been recently diagnosed or are unhappy with their current health status. However, this does not mean that the patient is fully ready to accept their condition or understands it fully.
The patient will be asked to write down inventory in the categories of physical, emotional, spiritual, social, recreational, intellectual, and environmental. The purpose of this is so the patient is able to search and reflect on where they’re currently at and where they would like to be.
From here, there are different techniques and models that a coach may use. One being the transtheoretical model, in which the patient will use stages to move through a behavior change.
At this point, the conversation is less about treatment and more about obtaining an awareness of their health risks, experience with a current illness or any symptoms they�re experiencing. The patient is welcome and encouraged to express their emotions openly. The health coach will move through these next 6 steps to help outline the patient’s treatment and see what stage they are at.
1. Precontemplation: the patient does not intend to take action in the foreseeable future
2. Contemplation: intending to start introducing healthy behaviors within the next 6 months
3. Preparation: patients are ready to take action in the next 30 days
4. Action: the patient has recently changed their behavior and intends to keep moving forward
5. Maintenance: the patient has sustained their behavior change for 6 months and intends to maintain the behavior change for more than six months
6. Termination: the patient has grown and is now self-aware of their behaviors and has no desire to return to their previously unhealthy behaviors
As we all know, values are formed starting in early childhood. These values are then later consciously re-evaluated and may change. It is important for the patient to work with the health coach so they properly understand their personal values. This allows the patient to get clarity and build self-awareness to make intelligent decisions and keep a balance in life.
Actually sitting down identifying values might be difficult, as many individuals do not think about them often. If this is the case, the health coach might help by asking questions like:
What is more important in your life: Beyond basic human needs, what must you have in your life to experience fulfillment?
Take this time and consider a meaningful moment: What was happening to you and what values were you honoring?
Consider a time when you were angry or upset: What were you feeling and, if you flip those feelings around, what value is being suppressed?
These questions aid in triggering times that the patient might not have been connecting to values. After the patient has identifies values, the health coach will work with the patient to select between 5-10 of their core values and then rank them in order of importance. From here, the patient is able to evaluate their values and proceed to determining goals.
Step 2: Determine Goals
Once a patient has identified their values, the health coach will shift their focus and have them brainstorm what they would like to focus on in their healing plan. This step is important because it will determine what they specifically want to change or improve. Some patients may feel unsure or are apprehensive, but allowing the patient time to journal or write out everything they want to accomplish, big or small, as well as the known steps or tasks, will help the patient get there.
When determining goals, the health coach will encourage the patient to create goals for multiple areas in their life. Some of these areas may include, health, family, relationships, and recreation. The health coach will encourage the patient’s goals by having them consider the following questions:
What do I want to achieve?
How will I achieve this goal?
Why do I want to reach this goal?
Who will I need to work with to achieve this goal?
What are the conditions and limitations to achieving this goal?
Based on the core areas and goals, the health coach will work with the patient to determine SMART goals. A SMART goal is a goal-setting technique that brings structure and trackability to goals. SMART goals stand for Specific, Measurable, Attainable, Relevant, and Timely. These create a verifiable trajectory towards a specific objective with clear milestones. By determining SMART goals, it clarifies how and when the goal will be achieved, rather than just stating a desire.
A health coach will help patients turn “I want to lose weight” into ” I want to lose 20 pounds to have more energy to play with my grandchildren. I will do so by exercising four times a week and eating less processed foods, and more fruits and vegetables. I will lose an average of two pounds every week for 10 weeks.”
By doing this, the health coach is working on a goal that immediately interests the patient and puts it into a way that is more attainable. The health coach can help the patient stay encouraged and motivated as they work together to achieve small successes, eventually leading to the patient being more willing to take on bigger challenges.
Using health coaching can be more beneficial than one might originally think. As one can see, they really go deep into one’s life and can help them in ways they might not have planned on originally. In the next article, the steps of building a plan for action and tracking progress and results will be discussed in great detail.
Changing your lifestyle can be difficult and does not happen overnight. Those who work with a coach to reach their goals are more successful and less likely to give up when things get difficult. Coaches are amazing for accountability, advice, health help, goal setting, and organizing expectations in a realistic timeline. Look at it this way: People use a wedding coordinator to help them oragnize food, timelines, expectations, etc. and that is for an event that lasts 1 day. So why are you not using a health coach to help you organize all of these same things for something that will last a lifetime? In addition you’re getting to help decide your future and gain a deeper undestanding of what lies ahead. How often do you feel more susceptible to pain, discomfort, and inflammation? Rheumatoid arthritis (RA) is an autoimmune disease that affects about 0.5 to 1.0 percent of the adult population throughout the world. Evidence suggests that the autonomic nervous system (ANS) plays a fundamental role in the management of an abnormal immune response and inhibition of inflammation. The ANS regulates cytokine production through the cholinergic anti-inflammatory pathway, including the efferent vagus nerve, the neurotransmitter ACh, and its receptors (?7 nicotinic ACh receptor, ?7 nAChR). �
Turmeric, or curcumin, has historically been utilized as a spice and a medicinal herb in India and China. Evidence suggests that curcumin affects diverse bioactivities. In recent years, a variety of research studies have shown that consuming curcumin considerably ameliorated collagen-induced arthritis (CIA). A clinical trial has shown that curcumin is a safe and effective natural remedy for RA patients. However, pharmacokinetic research studies have shown that its bioavailability can be very poor which raises the question, how does curcumin, or turmeric, produce an anti-inflammatory effect? �
Curcumin, the Gut-Brain Axis, and Inflammation
Researchers believe that they have found how curcumin, or the natural phenol compounds found in turmeric, can ultimately help reduce inflammation. Curcumin-containing turmeric is commonly given to horses by their owners due to its perceived anti-inflammatory properties. As a matter of fact, researchers have shown the ability of curcuminoids to reduce inflammation in aging horses. In one research study, researchers writing in the Journal of Neuroinflammation described their findings with rats that had collagen-induced arthritis to learn more about the performance of curcumin. The research team stated that several research studies had shown that taking curcumin by mouth reduced collagen-induced arthritis. �
During another clinical trial, the findings had shown that curcumin was safe and effective for patients with rheumatoid arthritis. However, the research study had also shown that its bioavailability was poor, which raised questions about how curcumin produced its anti-inflammatory effect. They then evaluated whether the gut-brain axis was involved in its therapeutic action. The researchers found that curcumin reduced collagen-induced arthritis through the gut-brain axis by controlling the function of the cholinergic system. The cholinergic system consists of nerve cells that utilize the neurotransmitter acetylcholine. It is associated with cognition, memory, selective attention, and emotional processing. �
The research study team believes that targeting the gut cholinergic anti-inflammatory pathway could be a promising treatment approach for rheumatoid arthritis patients as well as with other inflammatory disorders, such as irritable bowel syndrome, characterized by an imbalanced autonomic nervous system. Previous research studies also suggested that curcumin activated its effects in a gut-dependent way. The gut is a sensory organ, which sends signals from the lumen to the central nervous system, which in turn sends signals to peripheral tissues or organs. Surprisingly, recent research studies ultimately show that curcumin has regulatory effects on the gut microbiota in several circumstances and that the gut microbiota is involved in the development of arthritis. �It is possible that curcumin affects the cholinergic anti-inflammatory pathway through the gut-brain axis via modulation of the gut microbiota�, stated the research study team. �
How Turmeric Changes the Gut Microbiome
Further research studies are still required to continue to explain the turmeric paradox: efficacy despite poor bioavailability. One possible hypothesis as to how curcumin and turmeric change the gut microbiome, according to the first human clinical research study evaluating its effects, is that the gastrointestinal effects of the spice may have wider-reaching systemic effects. Previous research studies show that curcumin can considerably alter gut microbiota where these alterations of the composition and/ or metabolic activity of gut bacteria may in part explain the therapeutic benefits of curcumin. �
To demonstrate how curcumin alters gut bacteria in humans, a research study involved a group of people taking turmeric (3000 mg turmeric root plus 3.75 mg black pepper�derived extract of piperine alkaloid [BioPerine]), curcumin (3000 mg of curcumin [Curcumin C3 Complex] plus 3.75 mg BioPerine) or placebo, twice daily for 8-weeks. Utilizing gut microbial DNA sequencing it was found that both turmeric and curcumin changed the gut microbiota in a highly similar manner, with the research study team suggesting that curcumin may drive the majority of changes observed in turmeric-treated subjects. �
Overall, turmeric and curcumin increased bacterial species richness. Surprisingly, although the gut microbiota response was highly individual, there were several concordances in response to turmeric/ curcumin. �Responsive� subjects had a similar microbial signature involving uniform increases in species of polysaccharide-degrading and hydrogen-consuming bacteria. �
�This research study in healthy subjects has potentially raised more intriguing questions than it has fully answered and emphasizes the complexity of human intervention research studies intending to evaluate the effects of these potentially powerful herbal medicines,� stated the research study investigators. One hypothesis they suggest is that individual variations in turmeric absorption may result in the observed variations in prebiotic-like effects. �Future research studies that include a larger human cohort will clarify whether the responsive microbiota we identified here is representative and whether less prevalent response signatures in our data may be clearly defined with additional participants,� they concluded. �
If there is definitely a fundamental role of the gut microbiome in regulating individual responses to turmeric and/ or curcumin more research studies in this field could help support the understanding of its health benefits in the context of personalized nutrition, especially for rheumatoid arthritis (RA) patients, among patients with inflammatory health issues. �
Turmeric, or curcumin, is a powerful, natural remedy which has been demonstrated to have many health benefits, especially for brain health. Q: Dr. Jimenez, I read one of your articles about physical therapy and spinal stenosis exercises that focus on stretches for relieving pain. I was wondering if it was also possible to do aerobic exercise with a spinal condition and can you recommend a safe cardiovascular program?
I’m a 65-year-old with spinal stenosis, and I want to stay in shape. I try to ride a stationary bike for 20 minutes at least 2 times a week, but with my low back pain, I don’t always finish the workout.
How else can I stay in shape
A: I do recommend aerobic exercise for everyone, but especially for people with spinal conditions.
Aerobic exercise increases the blood flow to the body’s tissues, and people with high levels of cardiovascular fitness generally do better dealing with spinal problems.
However, before anyone with a spine condition or any medical condition for that matter, starts a wellness and fitness program, they should check with their primary caregiver, to clear the individual as fit to exercise.
Example: Someone with cardiovascular (heart problems) can have restrictions when it comes to certain types of exercise.
A physical exam will make sure your body is ready for exercise.

Low-impact aerobic exercise is recommended�
Walking
Swimming
These are excellent examples of low-impact aerobic exercise. They increase heart rate and are easy on the body.
Riding a stationary bike is another recommended form of low-impact aerobic exercise.
It can be tiring, but if recommended by a caregiver/therapist, then realize they did so for a reason/s to get you healthy.
